Benefits for Hourly U.S. EmployeesCompuware offers full-time hourly employees the opportunity to purchase company-sponsored benefits on a pre-tax basis. These voluntary benefits permit you to customize a benefit package to meet the needs of you and your family at group-rate prices. Full-time hourly employees maintaining a 30-hour/week work schedule will be eligible (on the first of the month following 90 days of employment) to select from the following benefit options: Medical/PrescriptionThe PPO medical plan offers you the option of utilizing the in-network providers or the freedom of choice to elect any physician. Premium costs are deducted from pre-tax dollars. If you elect to use in-network services, your annual deductibles will be limited to $1,000 per individual, and $2,000 for family coverage. After the deductible is met, your co-pay will only be 30 percent for most hospital and physician services, up to an annual out-of-pocket maximum, including deductibles, of $6,000 per individual coverage and $17,000 for family coverage. Covered services include hospital and physician services, emergency services, diagnostic, x-ray and lab. The in-network option is designed to hold costs down. A retail prescription drug plan is offered at 30 percent co-pay. If you elect to use out-of-network services, deductibles increase to $2,000 per individual and $4,000 for family coverage. The co-pay on most hospital and physician services increases to 50 percent with an annual out-of-pocket maximum, including deductibles, of $12,000 per individual coverage and $34,000 for family coverage. Prescription drugs require a 30 percent co-pay. DentalCompuware offers dental coverage for you and your family at competitive rates. Premium costs are deducted from pre-tax dollars. Annual deductibles are limited to $50 per individual or $150 for family coverage, with an annual maximum benefit of $1,000 per person. Coverage is based upon the type of services obtained:
In addition to offering a dental plan that encourages good dental hygiene, an orthodontia feature is also offered with 50 percent co-pay and a lifetime maximum of $1,000 per individual. Reimbursement AccountsThe Health Flexible Spending Account (Health FSA) reimburses you for expenses incurred by you and your dependents that are not covered by your medical or dental plan. You may elect to contribute a maximum of $2,000 annually on a pre-tax basis to cover anticipated expenses during the calendar year (January 1 through December 31). The Dependent Care Reimbursement Account (Dependent Care FSA) allows you to tax shelter dollars spent to provide day care services for children under age 13 disabled spouse/parent while you are at work. You may contribute up to $5,000 annually. 401(k)Compuware offers a 401(k) plan to employees who have completed one hour of service for Compuware. Contributions to the Plan can be made up to 75% of compensation on a pre-tax basis (up to the annual IRS limits) and 10% on an after-tax basis. Employees have access to their Fidelity 401(k) account virtually any time through the web and voice response system. There are no limits on the movement of funds from one investment choice to another. Employee Referral BonusAn employee may be eligible to receive a bonus for a referral that results in a professional or managerial level salaried hire. Value Options – Employee Assistance ProgramEmployees and their immediate family members may call toll-free for information and/or counseling on just about anything that makes work life and personal life difficult to balance. MetPay Group InsuranceCompuware offers MetPay, a voluntary benefit program that enables eligible employees to insure their cars, homes, boats, recreational vehicles and other personal property at special group rates through direct payroll deduction. Veterinary Pet InsuranceVeterinary Pet Insurance offers affordable policies that cover more than 64,000 medical conditions related to accidental injuries, illness, etc. for pets. Policy premiums can be paid through direct payroll deduction.
